a.  Application for Japanese Government Scholarship (must be signed by the applicant and be printed on both-sides). c. a letter of recommendation from a dean or a president of the home university addressed to the president of Chiba UniversityImmediately Try to ask Monica-san if the president of UMS can provide a recommendation letter for you. If he/she can not do that, get one from the dean at least.Ask your president at first! Need original copyThe recommendation letter should be send by FEDEX with the first item and reach to me until 21 Jan.
d. two pieces of your photos (the one taken within six months and 4.5 x 3.5 cm in size) . High resolution digital picture is acceptable.Digital photo should be sent to TK by e-mail ASAPSend four copies of the picture to TK until 21 Jan.
Prrof of your status of citizenshp and nationallity (for example, copy of passport, registration certificate for citizenship, etc)perhsps, passport would be the easiest way. Sned a high resolution scan of your passport to TK, ASAP.
f. academic transcripts (with English or Japanese translations attached) issued by the university which an applicant last attendedYou sent me the one for your BSC, but we need the one for your MSC. Immediately, Ask your university, and academic staff or private company to translate it.Copy and English translation is necessary. Attach a certificate for translation is better (you can ask your supervisor or academic staff).You can send me high resolution scan by e-mail (but you should keep the original with you, because it may be necessary for you in future)
g. a graduation certificate (or a letter that certifies your expected graduation date) or a proof of the degree (with English or Japanese translations attached)Immediately,Ask your university, and academic staff or private company to translate it.Original and English translation is necessary. i. a summary of a graduation thesis etc.Written in English is better. Note: the judgement committee in Chiba University will use this item for evaluation and selectiopn.Digital material is OK.
j. any documents that show an applicant’s language proficiency (Ex. scores of TOEFL, TOEIC, Japanese Language Proficiency Test, a Japanese score of Examination for Japanese University Admission for International Students, etc)You are planning to take TOEFL exam on 18th Jan. If you take this test, you will receive your score on in the beginning of Feb.. which perhaps will be in time for your 2nd selection by the judgement commiitee.
